Rates

*Initial Therapy and Diagnostic Assessment: $250.00 (75 min session) 

Note: This rate does not apply to individuals referred through LYRA

*Follow-Up Therapy Session Fee: $190.00 (50 min session)

Note: This rate does not apply to individuals referred through LYRA

*Follow-Up Therapy Half-Session: $95.00 (25 min session)

​

*Immigration Evaluation and Report: $1,200.00 flat rate (+ $150.00 fee for home visit, if desired)

  Please note that an evaluation for completion of Form N-648, extreme hardship, U-Visa, or VAWA consists of one to three face-to-face sessions. A U-Visa evaluation may be completed in one session (approx 2-4 hours of face to face time, records review, and report writing).

An immigration evaluation report is typically completed in 1-2 weeks. Expedited reports are available at an additional fee of $200.00 with a guaranteed timeframe of 2-3 days after the evaluation session(s).

*Surrogacy Evaluation: $1200.00

*Bariatric Surgery Assessment: $550.00 (flat rate)

*Spinal Cord Stimulator Candidate Assessment: $250/hr rate (evaluation approx. time= 3-5 hours)

**Please note, any case that results in a deposition or subpoena, is subject to a rate of $200/hr to include preparation time, court, and travel expenses**

PROFESSIONAL CONSULTATION: Dr. Olivares is available on a case-by-case basis for professional consultation at a fee of $50/30 min.
